Easy Choice Health Plan (HMO) is Medicare Advantage plan that includes Prescription drug coverage designed to make it easy for you to obtain your generic and brand name drugs at conveniently located network pharmacies.

Anyone with Medicare Parts A and B is eligible to join Easy Choice Health Plan (HMO).  You must reside in the health plan service area and receive all routine care from plan providers except in the case of emergent or urgent care.  If you obtain routine care from out of plan providers, neither Medicare nor Easy Choice Health Plan (HMO) will be responsible for the costs.

Easy Choice Health Plan (HMO) is a Medicare Advantage organization with a Medicare contract that is renewed annually. Coverage beyond the end of the contract year is not guaranteed.

Humana Prescription Drug plans include the PDP Standard, the PDP Enhanced, and the PDP Complete. The PDP Standard is the basic plan if your need for prescription drugs is minimal. PDP Standard provides basic drug coverage for people with Medicare. The PDP Enhanced plan is for those who are considered “in the middle” and need to take care of a few chronic conditions.
