Easy Choice Health Plan (HMO) is a Medicare Advantage Prescription Drug Plan for people living in select California Counties who have both Medicare Part A and Part B. Our goal is to make healthcare Easy for you! With “one stop service” for doctors, hospitals and prescription drug needs, more benefits than Original Medicare, including services to help keep you healthy. Easy Choice Health Plan (HMO) is Medicare Advantage HMO plan that includes prescription drug coverage designed to make it easy for you to obtain your generic and brand name drugs at conveniently located network pharmacies. Anyone with Medicare Parts A and B is eligible to join Easy Choice Health Plan (HMO). You must reside in the health plan service area and receive all routine care from plan providers except in the case of emergent or urgent care. If you obtain routine care from out of plan providers, neither Medicare nor Easy Choice Health Plan (HMO) will be responsible for the costs. Easy Choice Health Plan (HMO) is a Medicare Advantage organization with a Medicare contract that is renewed annually. Coverage beyond the end of the contract year is not guaranteed.2015 Easy Choice plans are available in the following counties; Alameda, Fresno, Los Angeles, Orange, Riverside, San Bernardino, Santa Clara, San Diego, San Joaquin and Stanislaus. |
